Job Details

Qualifications:

  • RN License in Colorado
  • 1 year of nursing or case management experience required.

In this role, you will:

  • Advocate for patients
  • Collaborate closely with interdisciplinary healthcare partners, facilities, and families
  • Ensure a seamless care experience from admission through discharge and beyond
  • Connect patients to resources that support and enhance positive outcomes
  • Utilize clinical expertise, discretion, and independent judgment
  • Assess and reassess patient needs
  • Facilitate care coordination and utilization management
  • Serve as a patient advocate throughout the healthcare journey
  • Assure that medical appropriateness criteria are met for both status and level of care
