Job Details
We are seeking a passionate and experienced Registered Nurse to join our team as a Permanent Registered Nurse for Home Health Hospice in the Medical industry. As a member of our team, you will have the opportunity to work with patients in their homes and provide compassionate care during their final stages of life. You will be responsible for providing skilled nursing care to patients, coordinating care with other healthcare professionals, and ensuring that patients receive the highest quality of care.

Responsibilities:
- Provide skilled nursing care to patients in their homes
- Assess and monitor patients' physical and emotional status
- Develop and implement care plans in collaboration with other healthcare professionals
- Administer medications and treatments as prescribed by physicians
- Educate patients and their families on disease management and end-of-life care
- Coordinate care with other healthcare professionals, including physicians, social workers, and therapists
- Document patient care and progress in accordance with regulatory and agency requirements
- Participate in interdisciplinary team meetings to review patient care plans and make recommendations for adjustments as needed
Qualifications:
- Active RN license in the state of practice
- Minimum of 2 years of experience as a Hospice RN, Home Health Nurse, or Certified Home Health RN
- Strong clinical skills and knowledge of end-of-life care
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong organizational and time management skills
- Comfortable working with patients in their homes and in a variety of settings
- Willingness to work flexible hours, including evenings and weekends
