Job Details

  • Perform holistic assessments and craft individualized hospice plans of care

  • Coordinate closely with physicians, social workers, chaplains, and aides to deliver seamless service

  • Teach patients and families about the disease process, symptom control, and end‑of‑life decisions

  • Monitor conditions, update care plans, and communicate changes to the medical team

  • Document all visits and interventions to meet federal, state, and accreditation standards

  • Guide and evaluate Hospice Aides/LPNs to ensure top‑tier care

  • Participate in IDG meetings, quality‑improvement projects, and staff education

Skills You Need
  • Active RN license and at least one year of clinical experience (hospice a plus)

  • Sharp assessment, critical‑thinking, and time‑management abilities

  • Empathetic communicator who builds trust with patients, families, and teammates

  • Valid driver’s license and reliable, insured vehicle for home visits
