Starting Salary: $100k-$119K per year + annual bonuses up to $5,000
Schedule: Monday to Friday, No Weekends, No Calls
Hours: Flexible, 9 am-5 pm or 7 am-3 pm
Locations: Brooklyn, Manhattan, and Queens

The Home Care RN Case Manager sees patients in their homes.
What you will be doing as a Registered Nurse:
Qualifications:
* Must have an active RN (Registered Nurse) license in the State of New York.
What you will be doing as a Registered Nurse:
- Provide case management services for homecare patients
- Complete initial assessments, OASIS, recertifications
- Creates and implements a plan of care
- Collaborates with the interdisciplinary team and coordinates patient care
- Educates patients and families
