Qualifications:
- Conduct comprehensive health assessments, including obtaining medical histories and vital signs
- Identify and prioritize patient needs based on presenting conditions
- Collaborate with physicians and other healthcare professionals to develop and implement individualized treatment plans
Care Coordination and Management:
- Coordinate patient care across various healthcare settings and services
- Manage chronic disease patients through education, monitoring, and follow-up
- Assist in care transitions and referrals to specialists or community resources
Patient Education:
- Provide patient and family education on health conditions, treatments, and preventive care
- Offer guidance on medication management, lifestyle modifications, and self-care strategies
Clinical Procedures:
- Administer medications and vaccinations as prescribed
- Perform routine clinical procedures such as wound care, injections, and basic diagnostic tests
- Assist with minor office procedures as needed
Documentation and Quality Improvement:
- Maintain accurate and timely electronic health records
- Participate in quality improvement initiatives and contribute to the development of best practices
- Ensure compliance with regulatory requirements and clinic policies
Qualifications:
- Current New Hampshire Registered Nurse (RN) license
- Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) considered
- Minimum of 1-2 years of nursing experience, preferably in primary care or community health
- Basic Life Support (BLS) certification
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal skills
- Proficiency in electronic health record (EHR) systems
- Commitment to serving diverse and underserved populations
