Job Details
We are seeking a Registered Nurse to provide comprehensive nursing care, patient education, triage services, care coordination, chronic disease management, and clinical support across outpatient practice settings.
Key Responsibilities:
Perform patient assessments and triage
Administer medications and treatments
Support chronic disease management initiatives
Conduct nurse visits
Coordinate diagnostic testing and follow-up care
Educate patients and families
Collaborate with providers and interdisciplinary teams
Support population health and quality initiatives
Required Qualifications
Active Connecticut RN License
Associate Degree in Nursing
CPR Certification
