Job Details

Full‑time, field‑based Hospice RN Case Manager (3-4 patients a day)
Perform physical, psychosocial, and health system assessments to develop individualized plans of care
Develops and implements a nursing plan of care and makes recommendations for care based on patients' needs.
Requests written orders from physicians for changes in the patient plan of care as needed.

Requirements
Graduate of an accredited School of Nursing (BSN preferred)
Active New York State RN license; CPR‑BLS preferred
Minimum of two years of nursing experience (Medical‑Surgical or Community Health; hospice or oncology preferred)
