Job Details
- Serve as a key intermediary between hospital staff, patients, and their families to ensure clear and effective communication and coordination of hospice care.
- Conduct thorough chart assessments of patient cases and bedside evaluations to determine the clinical picture and hospice eligibility
- Collaborate with facility clinical team and hospice physicians to make informed decisions regarding patient care
- Successfully facilitate numerous hospice admissions ensuring seamless transitions for patients and their families
- Provide critical support and education to enhance patient and family understanding of hospice care, leading to improved satisfaction and care outcomes
- Oversee the coordination of admission assessments and the procurement of durable medical equipment, ensuring that all patient needs are met promptly and effectively
