Home Care Field / Visiting RN seeing patients in their homes.
What you will be doing:
· Complete initial assessments, OASIS, recertifications
· Creates and implements a plan of care
· Collaborates with the interdisciplinary team and coordinates patient care
· Educates patients and families
Skills you need:
· RN license in NY
· Fluent in Mandarin, Cantonese, Spanish, Russian, or Creole
What you will be doing:
