Job Description:
We’re seeking a licensed Speech-Language Pathologist to join our care team and provide exceptional therapy services in accordance with each individual’s care plan. This role is ideal for someone who thrives in collaborative settings and is passionate about person-centered care.
Responsibilities include:
Evaluate, diagnose, and treat speech, language, cognitive, and swallowing disorders
Develop and implement individualized treatment plans
Collaborate with nursing, rehab, and social work teams to ensure coordinated care
Maintain accurate clinical documentation and progress notes in accordance with state and federal regulations
Participate in team meetings and patient case reviews
Qualifications:
Master’s degree in Speech-Language Pathology
Current state licensure (or eligibility)
Certificate of Clinical Competence (CCC-SLP) preferred, but CFs welcome to apply
Experience in skilled nursing or geriatric care is a plus
